This integrative narrative (qualitative) review synthesizes, from 2006 to 2024, evidence on lactose intolerance across three intersecting layers: biology (LCT persistence/hypolactasia and lactase physiology), intestinal ecology (microbiota, probiotics and prebiotics), and everyday life (nutrition and psychosocial aspects). Ten studies were included. Findings support person-centred care: individualized dietary adjustment, judicious lactase use, and microbial modulation, while safeguarding calcium/vitamin D status and quality of life. Recurrent barriers—diagnostic heterogeneity, symptom overlap, and strain-dependent effects—call for outcome standardization, microbiome-based stratification, and precision-nutrition strategies.
