Delayed Osseointegration of Dental Implants: Clinical Experience Report with Cases of Spontaneous Recovery after Initial Failure

Abstract

Introduction: Osseointegration is considered essential for the success of dental implants. Classically, early failure is indicative of the need for immediate removal. However, this experience report describes cases in which implants initially considered unsuccessful presented delayed osseointegration.

Experience report: Three different patients were followed after the placement of implants that demonstrated clinical and radiographic signs of early failure (lack of stability, peri-implant bone loss, absence of radiographic signs of osseointegration). In all cases, the patients chose not to perform immediate removal of the implants. After periods ranging from 8 months to 2 years, complete osseointegration of the implants was observed, with satisfactory clinical stability and radiographic signs.

Discussion: Experience suggests that there may be rare cases of delayed osseointegration, challenging established clinical protocols. Local biological and metabolic factors and even initial interpretation errors may be involved.

Conclusion: Although removal of failed implants remains the standard procedure, the possibility of delayed osseointegration should be considered in specific contexts and with careful monitoring.
