Injectable platelet-rich fibrin for the treatment of periodontal intrabony defects: A systematic review

Esta revisão sistemática teve como objetivo avaliar a evidência acerca da eficácia clínica e radiográfica da Fibrina Rica em Plaquetas Injetável (i-PRF) como adjuvante em procedimentos aumento ósseo, comparativamente com outros biomateriais ou PRF. Foram incluídos estudos do tipo Ensaios Clínicos Randomizados (ECRs) que abordavam o uso de i-PRF ou "Sticky Bone" em comparação com enxertos ósseos isolados (DFDBA, HA, Xenógeno) ou PRF convencional. A busca foi realizada nas bases de dados PubMed/MEDLINE, Scopus, Web of Science, Embase e Cochrane Library e em outras fontes até Outubro de 2025. O risco de viés dos estudos incluídos foi avaliado por meio da ferramenta RoB 2.0 (Risk of Bias 2.0) da Cochrane. A síntese dos dados foi realizada de forma narrativa, agrupando os estudos por tipo de intervenção e desfechos avaliados. Foram incluídos 5 estudos, selecionados a partir de 646 artigos iniciais, totalizando 135 participantes. Quatro dos cinco estudos (80%) demonstraram desfechos clínicos (redução de PPD, ganho de CAL) e radiográficos (preenchimento ósseo) estatisticamente superiores para o grupo que utilizou i-PRF. Um estudo (20%) não encontrou diferença significativa entre os grupos. Três estudos também relataram melhorias significativas na manipulação do enxerto (formação do "Sticky Bone") com o uso da i-PRF. evidência atual sugere um potencial benefício da i-PRF na otimização dos resultados da regeneração óssea. Contudo, a heterogeneidade metodológica e a baixa quantidade de ECRs limitam a emissão de uma recomendação clínica robusta. Esta revisão foi registrada na plataforma PROSPERO (International prospective register of systematic reviews).
